Appointments

1. If, for any unforeseen reason, you are not able to keep your scheduled appointment or wish to reschedule your appointment, please notify the office at least one day in advance so that we can accommodate other patients and ensure we can secure a rescheduled appointment as per your needs.

Insurance Coverage

2. All patients are required to present an insurance card at the time of each visit and inform our staff if there are any changes to the coverage, address, or contact information so we can keep your information updated and accurate.

Patient Dues

3. Please promptly pay all charges assigned to you by your insurance provider. If you have any questions about your insurance coverage or assigned deductible fees -Please contact your insurance provider. *Late payments may incur additional charges.

Referrals

4.Please provide at least one week’s notice for all referrals.

Student or Patient Health Forms.

5. Please submit all paperwork with instructions at least one week in advance – to avoid any possible delays.
